• Your GP may refer you to see an osteo, physio or exercise physiologist with a Chronic Disease Management Plan.

• You may be eligible for up to 5 treatments under this plan.

• Standard Medicare rebates apply after full payment on the day of service. This can be done on the spot at reception.
